Problems Associated With Ejaculation



The process by which the semen squirts out from the penis of the men during a sexual orgasm is known as ejaculation. The problems associated with ejaculation are:

Premature ejaculation where the man ejaculates very quickly at the time of sexual intercourse. The normal time taken by men for ejaculation after the insertion of the penis into the vagina is about four to five minutes. So anyone who ejaculates before four minutes is considered to have the problem of premature ejaculation.
Delayed ejaculation where the person does not ejaculate during the sex, despite his wish and normal erection. Some of the causes of delayed ejaculation include diabetes and hypertension.
Retrograde ejaculation occurs when the sperms moves back to the bladder instead of coming out through penis. Even though the person will experience orgasm, there will be no semen associated with the orgasm.

Symptoms

The major symptom associated with premature ejaculation as well as delayed ejaculation is psychological feeling that he has a problem.

If a person ejaculates while masturbating but he is not able to maintain ejaculation while having sexual intercourse, which might be due to a feeling of guilty or anxiety because of having sex.

The other symptom associated with retrograde ejaculation is the flow of cloudy urine the first time the person urinates after sex.

In rare cases, a man may find blood in his semen. This is known as haematospermia. This may be due to the inflammation of the prostate or urethra as a general reaction to an infection like swelling.

The other causes of presence of blood in semen are:

  • traumatic injuries in genital area
  • sexually transmitted infection like chlamydia or gonorrhoea
  • prostate cancer

Causes

Premature ejaculation is of two types:

the primary premature ejaculation where the person had always had premature ejaculation from he being sexually active
the secondary premature ejaculation where ejaculation was normal earlier

The psychological factors for primary premature ejaculation are:

  • Conditioning due to masturbation or having sex during teenage.
  • any traumatic events related to sex at a very young age
  • if the person has been taught that sex is appropriate only under certain cases

Secondary premature ejaculation can occur due to

  • Psychological problems like
  • Problems or conflicts that remain unresolved in a emotional and sexual relationship
  • Anxiety while having a sexual relation
  • Stress
  • Depression
  • Physical problems like
  • Multiple sclerosis
  • Diabetes
  • Disease of the prostate
  • Hypertension
  • Hypothyroidism
  • Excessive consumption of alcohol

Delayed ejaculation may be due to depression, stress etc or physical problems like diabetes, injuries to the spinal cord etc.

Retrograde ejaculation occurs as a result of the damage of the muscles or nerves around the bladder’s neck.

Treatment

For premature ejaculation:

self care techniques like masturbating two hours before sex and use of thicker condom to reduce the sensation
couples therapy where the squeezing and stop and go techniques are taught
use of over the counter drugs like local anaesthetics in combination with the stop-go or squeeze techniques.

For delayed ejaculation, medication combined with psychological therapies help in improving the situation.

Only if the men having retrograde ejaculation want to have a baby, artificial insemination of IVF is used.
